What to write in a home blood pressure record
A home record is easier to discuss when it shows more than a list of numbers. Include when and how you measured, keep the original readings together, and follow the measurement schedule recommended by your healthcare professional.
Start with the measuring device and instructions
DailyHealth does not measure blood pressure. Use an appropriate external monitor and follow its instructions. The American Heart Association recommends an automatic, cuff-style upper-arm monitor for home use and advises people to discuss home monitoring with a healthcare professional. A professional can help check the device, cuff fit and technique.
Keep each entry easy to interpret
For each measurement, record the date, time and both values exactly as shown by the device, including the units if displayed. Note which arm you used if your clinician has asked you to use a particular one. If your monitor reports pulse, keep that value clearly labeled so it is not confused with either blood pressure number.
Also note anything that makes the measurement different from your usual routine, such as taking it soon after activity or feeling unwell. The note is context, not a diagnosis.
Use a repeatable setup
For more comparable readings, follow your clinician's instructions and the monitor manual. The American Heart Association advises sitting quietly before measuring, using a supported seated position, keeping the arm supported at heart level, and taking readings at consistent times. It also recommends recording multiple readings as instructed. Do not change your care plan based on an app or a single entry.
Bring the log to your appointment
Share the record with your healthcare professional, including readings that seem unusual and notes about the circumstances. Ask how often you should measure, what to do when a result is outside the range they expect for you, and whether they want the monitor brought to an appointment. Do not stop or change prescribed medicines based on a home log without professional advice.
What DailyHealth can help organize
DailyHealth lets you manually enter blood pressure values that were measured with an external device, then revisit the saved history. It does not take a blood pressure measurement through your phone.
